Clearance denied, revoked, suspended, etc. soley based on Poly

Has anyone seen officially where a person currently holding a clearance was denied, revoked, suspended, etc. soley based on another agency's claim of failed poly and there were no admissions of any wrongdoing?

Is there a case where someone was given a DOHA Statement of Reasons (SOR) in which the reason was a failed polygraph? I've searched and cannot find one.
