Past employment on SF85P

Hello,
I have this question about past employment on the SF-85P form. I just graduated from college and now at age 22. When I was 18 and 19, I worked on various jobs on campus in a very short time. I only reported tax to one because the amount was too little and by law im excepted, however, state tax were deducted from my pay check. Some of the jobs I left without notice so Im worried what my supervisor will answer on the form so I do not really want to include them. My question is, for those jobs that i do not have a W-2 form on, should I still put it down on the sf-85P??

Standard Form 85P is a Questionnaire for Positions of Public Trust. Those seeking positions of public trust have an ethical obligation to be honest in completing their applications. The instructions for the form say nothing about omitting jobs for which you do not have a W-2.

I actually called my university to find out my employment history. I was told that one of the job now has a new manager and the other one showed that I left because it was a tempt position. Should I include those jobs that I did not get a W-2 form and the supervisors will not remember anything about me after all?
and Will the investigator check my social to find all my employment history (I know they will pull out my credit report for sure)?

Another concern I have is there were this one job (in the supermarket) I worked for one week and left because I had to go home to finalize my study abroad trip.
Will I get denied because of my past employer/s (all were part time) deny to re-employ me??

Of course, if your BI involves investigators actually going out and trying to dig up dirt on you (the real purpose of the BI), then everything in your life becomes virtually an open book. :(
