How do you appeal an FBI polygragh?

I received the letter stating I did not pass my polygraph. Does anyone know how to appeal and request a retest? I know the odds are against me getting a retest or passing if I get one but I figure its worth a try.

Any one know what information to include besides the reason for the appeal, name, date, & location of test, SSN?
Do I send the letter to the person that sign the letter?
I dont want to leave out any neccesary information.
Any info would be appreciated.
