Although we are a fairly small team, we quality control every polygraph exam we run (crim specific and screening).  We believe this to be an important part of a good polygraph operation.  And yes, there are times Q.C. will non-concur with the examiner.  When that happens we run the test again..

Last week a research paper was published exactly on this topic (polygraph QA). Their major point is building an AI 2nd opinion for screenings.
They also make several additional points as they go, like:
- human QA is not perfect since examiner errors happen in part because the case is complicated, so human QA is likely to make the same mistake
- experimental lower bound for examiner error rate...
