suppressed produced this mock set of classified information cover
sheets, modeled on the official versions discussed above.  See:

These cover sheets would be appropriate for much of what is taught at the Department of Defense Polygraph Institute (http://www.dodpoly.army.mil).

I can think of a few other organizations and agencies that could fit these into their program quite easily :D
