You'll generally know if you failed a pre-employment polygraph "test" because you'll be subjected to a post-test interrogation during which the polygraph operator will badger you for admissions.
If you were not treated to a post-test interrogation, that's a good indication that you either passed or, at worst, the charts were scored as "inconclusive."
Is it always a bad sign if the examiner asks questions or requests clarifications while the chart isn't "flowing" ?

I just took poly today. Just wondering if polygrafer should tell the applicant either they passed or failed or should it be HR' job to do so. Is there indicator that we could see right away after the test for passing of failing.
Please advise.

I supposed it depends on where you applied for work. Generally, the polygrapher doesn't provide a concrete answer as to whether you "passed" or "failed". (in no small part, due to the fact that polygraphy is alchemy).
HR usually won't indicate whether or not the polygraph results obstructed your employment.
