Twoblock is correct. The form was drafted by the Missouri Union of Law Enforcement (MULE) and is available here: http://www.mule57.org/articles/polygrap.htm

I think you maybe talking MULE. It's an organization representing law enforcement in Missouri. I remember seeing it on this site, I believe, but don't remember where. If my recollection is accurate, it is a contract preventing employment polygraph results to be used in a criminal investigation. This document is to be presented to the polygrapher. If the polygrapher won't sign it, then the officer refuses to take the exam.
