Yes, I have heard of NSA applicants failing the polygraph without making any disqualifying admission.

Unfortunately, we don't have the data that would be necessary to answer your questions. However, it seems that it is quite common for NSA applicants who make no disqualifying admissions to initially be told by their polygraphers that they are "having problems" and to be grilled for admissions, and then to ultimately be brought back for one or more "re-tests." But we don't know what percentage of NSA applicants who make no disqualifying admissions pass.

And while we don't know what the overall NSA pre-employment polygraph pass rate is, either, rates of about 50% are not unusual at other agencies.
